According to Movie News Guide, some predictions state that Black Leg Sanji is definitely a Tenryubito, or part of the World Noble families in the "One Piece" world.
Sanji's mysterious family background came out first after new wanted posters for the entire Straw Hats Pirates, including Sanji, came out following the defeat of Shichibukai Donquixote Doflamingo. In the posters, while most of the crew received increased bounties on their heads, Sanji's poster changed – his previous "Dead or Alive" post has been amended to "Wanted Alive Only."
The industry follower said that a change in wanted status can only be done by individuals or families powerful enough to oppose the Marines.
Meanwhile, the report also alluded to Sanji being a member of a powerful Mafia household, instead of a Word Noble. Allegedly, Sanji's supposed family name – Vinsmock – is a hint already. "Vin" is French for wine, and smoke alluded not only to Sanji's preference for cigarettes but also his family background. This might also be why Big Mom chose Capone "Gang" Bege (another Mafia-like character) to capture Sanji.
Moreover, since the closing pages of the last chapter had Bege, Sanji, and some of the captured Straw Hat Pirates talking inside Bege's body (as an effect of Bege's Devil Fruit powers), observers have also predicted that succeeding chapters will also shed light on why Nami and the others were let go, with Sanji the only one carted away by Bege.
